PSP Format And reinstall OS

i Had PSP E1004 with v6.60 firmware and a month ago the OS got screwed. It wont let me change name,wont open saved games,wont go to the YES option when pressing the PSP button to quit games. So i tried updating the OS to 6.61 firmware and thought it might work. Tried Factory reset as well but all in vain. So can anyone please tell me how to format and reinstall 6.60 OS or any other way which will bring my PSP to normal state. Any help will be greatly appreciated. Thanks
