The Confirmation Code You Entered Was Incorrect

I’m trying to post a Game Request in the appropriate forum, and I keep getting the error message “The Confirmation Code You Entered Was Incorrect” – but the place for the confirmation code is blanked out, with no code showing, and no box to type a code in. It’s just smooth blue background colour. Help?
